If your child will be participating in any sport here at Clementon Elementary School you MUST complete all MANDATORY yearly forms for athletics that can be located by doing the following on our website:

Scroll down to Forms and Information Section

Go to Athletics

Open Annual Athletic Pre-Participation Examination Form

Print (Doctor and Parent MUST Complete Form)

Open Health History Updates Questionnaire

Print (Parent MUST Complete Form)

Open Concussion and Head Injury Fact Sheet

Print (Parent MUST Complete Form)

Sudden Cardiac Death in Young Athletes Sign-Off

Print (Parent MUST Complete Form)

Once you have all the above forms completed please submit them to the school nurse.  Students cannot participate until all four forms are returned before the sports begins.
